Horehound — Benefits Deep Dive
White horehound — Marrubium vulgare, a woolly grey-green bitter in the mint family, and not to be confused with black horehound (Ballota nigra), a different plant with a different traditional use — is one of the very few folk cough remedies that survived intact into the modern confectionery aisle. Horehound candy and amber lozenges are a genuine piece of living folk pharmacy, still made, still sold, still bitter. Its marker constituent is the labdane diterpene marrubiin, alongside marrubenol and a supporting cast of flavonoids and bitter principles.
And here is the fact that belongs at the top rather than buried in a footnote: the United States Food and Drug Administration concluded that horehound had not been shown to be effective as an over-the-counter cough and cold ingredient, and it was removed from the approved monograph list. That is why horehound drops today are sold as candy or as dietary supplements rather than as cough medicine. It is a documented negative regulatory finding on one of the most traditional remedies in the Western repertoire, and almost nobody writing about horehound mentions it.
The interesting part is that this does not contradict Europe, where horehound herb holds a traditional-use herbal registration covering cough and its use as a bitter digestive. A traditional-use registration explicitly does not require efficacy trials — it requires a long documented history of use, plausible safety and plausible pharmacology. Once you know that, the American and European positions stop looking like a disagreement and start looking like answers to two different questions. These four deep dives work through the regulatory story, the better-founded bitter-digestive use and the awkward modern research that complicates it, the chemistry of marrubiin and its rodent metabolic file, and the specific safety considerations — blood pressure, pregnancy, dose, and the sugar in the candy.
Horehound for Cough, and Why the FDA Removed It
The documented regulatory finding first: not shown effective as an OTC cough and cold ingredient in the United States, which is why the drops are candy now. Then why Europe's traditional-use registration for cough is consistent with that rather than contradicting it, what the lozenge is probably actually doing, and why a sweet viscous vehicle plus honey explains centuries of satisfied users without marrubiin doing anything.
Horehound as a Bitter: Digestion and Appetite
Horehound's better-founded use. Intense bitterness, a recognised cephalic-phase mechanism, bitter taste receptors throughout the gut, and a registered European indication for loss of appetite and mild dyspepsia. Plus the unresolved wrinkle: human studies of bitter compounds have shown reduced appetite and slowed gastric emptying, which does not sit comfortably with the traditional appetite-stimulant framing.
Marrubiin: Horehound's Chemistry and Metabolic Research
The labdane diterpene behind the bitterness, why drying converts premarrubiin into it, and the marrubenol finding that identified an L-type calcium channel block. Then the metabolic file — glucose, lipids, pain, inflammation — read at its real tier: rodent and in-vitro, with no adequate human trials, and a rodent hypoglycaemic result that is a caution rather than a diabetes benefit.
Horehound Safety: Blood Pressure, Pregnancy and Dose
Hypotensive and possibly arrhythmogenic effects at higher doses, so caution with antihypertensives and in cardiac disease; a plausible hypoglycaemic interaction with diabetes medication; pregnancy avoidance on the emmenagogue tradition; gastric irritation from the bitters. And the small concrete point nobody says: horehound candy is mostly sugar, which matters for the diabetic reader reaching for a natural cough remedy.

- Identity. White horehound, Marrubium vulgare, Lamiaceae. Square stems, wrinkled woolly grey leaves, whorls of small white flowers, intensely bitter. Not black horehound, Ballota nigra, which is a separate plant traditionally used as a mild sedative and for nausea.
- Marker chemistry. Marrubiin, a labdane diterpene lactone, formed largely from premarrubiin during drying; plus marrubenol, flavonoids including luteolin and apigenin derivatives, the phenylpropanoid verbascoside, tannins and a modest essential oil. Tier: well characterised chemistry.
- Cough, United States. Tier: NOT SHOWN EFFECTIVE. An explicit regulatory determination that horehound is not generally recognised as effective as an OTC cough and cold ingredient.
- Cough, Europe. Tier: traditional use only. Registered as an expectorant for cough associated with a cold, on the strength of long use rather than trials.
- Bitter digestive. Tier: traditional use only, with an established general mechanism. Registered for temporary loss of appetite and mild dyspeptic complaints; the cephalic phase and gut bitter receptors are real physiology, but the direction of effect on appetite is contested in human studies.
- Glucose and lipids. Tier: preliminary, animal only. Horehound infusions lowered glucose and improved lipids in diabetic rodents. Not a diabetes treatment; relevant chiefly as an interaction caution.
- Blood pressure and vessels. Tier: preliminary, mechanism named. Marrubenol acts as an L-type calcium channel blocker; extracts relaxed vessels and lowered blood pressure in hypertensive rats. Again a caution, not a treatment.
- Pain and inflammation. Tier: preliminary, rodent. Marrubiin reduced experimental oedema; extracts and marrubiin derivatives showed analgesia in animal models.
- Human trials. Tier: none adequate for any indication. This is the defining feature of horehound's evidence base.
- Main cautions. Pregnancy and breastfeeding; antihypertensive, antiarrhythmic and glucose-lowering medication; cardiac disease; active ulcer, gastritis, marked reflux or gallstones; Lamiaceae allergy; nausea and purgation at large doses; and the sugar load of the candy.
Research: The Regulatory Divide

The mechanism is solid; the direction of effect is not. This is the most intellectually honest corner of horehound's file.

Research: Safety, Pregnancy and Interactions
