Stevia and Blood Sugar
Stevia is the odd one out in this section, because it is not a remedy at all. It is a substitution. Steviol glycosides — the sweet compounds extracted from Stevia rebaudiana leaves — are not metabolised to glucose, so they raise blood sugar essentially not at all. The benefit is whatever sugar they displace, and nothing more.
Category: substitution. Framing it as a herb that lowers blood sugar gets the logic backwards.

Why the Distinction Matters
Every other entry in this section is asked to do something: activate a pathway, slow absorption, stimulate insulin. Stevia is asked only to not do something. It provides sweetness without carbohydrate, so a drink sweetened with it produces no glucose excursion where the sugared version would have produced a large one.
That is genuinely useful and it is entirely conditional on the swap. Stevia added to a diet that was not going to contain the sugar anyway does nothing whatsoever. The benefit lives in the difference between two versions of the same drink, not in the leaf.
What the Trials Show
Zare and colleagues' 2024 meta-analysis in Diabetes & Metabolic Syndrome pooled the human evidence and found stevia consumption associated with a significant reduction in blood glucose (weighted mean difference −3.84 mg/dL; 95% CI −7.15 to −0.53; P = 0.02), with larger effects in people with higher BMI, diabetes and hypertension.
Two things about that result. First, the authors graded the certainty of the evidence as low, which is their own assessment and should be reported alongside the number rather than buried. Second, a reduction of about 3.8 mg/dL is small in absolute terms — consistent with what you would expect from displacing some sugar, and not evidence of a pharmacological action.
The most defensible reading is the simple one: stevia is a way to remove sugar without removing sweetness, and the measured benefit is about the size that removing some sugar would produce.
Practical Notes
- Read the packet. Many tabletop "stevia" products are mostly dextrose, maltodextrin or erythritol with a little steviol glycoside for sweetness. Dextrose and maltodextrin are glycemic. The stevia is not the problem; the bulking agent is.
- Whole leaf and refined extract are different products with different regulatory status. Highly purified steviol glycosides are what safety assessments cover.
- Taste is the practical limit. Stevia carries a bitter, liquorice-like aftertaste for many people, and no amount of metabolic reasoning overcomes not wanting to drink it.
Cautions
- It lowers blood glucose, and so does your medication. The effects add. With insulin or a sulfonylurea this is a hypoglycemia risk, and dose changes belong with the prescriber — see hypoglycemia awareness and prevention.
- Sugar alcohols in blended products — erythritol, xylitol and their relatives — commonly cause bloating and diarrhoea at modest intakes.
- Measure, do not assume. Adding a supplement without measuring means you learn about an interaction from a symptom rather than from a number.
