Holy Basil for Blood Sugar
Holy basil (Ocimum sanctum, tulsi) occupies a different position from the other herbs here. It is classed as an adaptogen, and the case for it in blood sugar runs partly through stress physiology rather than directly through insulin — which makes it plausible for a particular kind of person and largely irrelevant for others.
Evidence tier 2: a small, positive, thirty-year-old human trial, and not much modern work behind it.

The Human Trial
Agrawal, Rai and Singh ran a randomised, placebo-controlled, single-blind crossover trial of holy basil leaves in people with non-insulin-dependent diabetes, published in the International Journal of Clinical Pharmacology and Therapeutics in 1996. The results were clear within the trial: fasting blood glucose fell by 21.0 mg/dL (95% CI −31.4 to −11.2, P < 0.001) and postprandial glucose by 15.8 mg/dL (95% CI −27.0 to −5.6, P < 0.02) — reductions of 17.6% and 7.3%. The authors suggested basil leaves might be used as an adjunct to diet and drug treatment in mild to moderate diabetes.
Those are respectable numbers from a properly randomised crossover design. They are also from a single small study, single-blinded rather than double-blinded, published in 1996, and the subsequent literature has not built on it the way it should have. Searches of the clinical databases return very little modern controlled work on tulsi and glycemic endpoints.

The Cortisol Route
Holy basil's adaptogenic classification points at a mechanism the other herbs on this list do not have. Cortisol raises blood glucose — that is one of its jobs, mobilising fuel for a threat — and chronically elevated cortisol from sustained psychological stress or poor sleep contributes to elevated fasting glucose and to insulin resistance.
If a herb genuinely moderates the stress response, it can lower blood sugar without touching insulin signalling at all. That makes holy basil a reasonable thing to consider for someone whose glucose control is visibly worse during stressful periods or after bad sleep, and a poor bet for someone whose problem is dietary. It is one of the few cases in this section where who you are should change the answer.
Dosing and Forms
Tulsi is most often taken as a tea from dried leaf, or as a standardised extract at roughly 300–600 mg a day. Doses in the trial literature vary and are not standardised, so no precise recommendation is well-founded. The tea is the traditional form and is pleasant, which matters for something taken daily.
Cautions
- It lowers blood glucose, and so does your medication. The effects add. With insulin or a sulfonylurea this is a hypoglycemia risk, and dose changes belong with the prescriber — see hypoglycemia awareness and prevention.
- Some evidence of antifertility effects in animal studies at high doses; avoid therapeutic doses when trying to conceive and in pregnancy.
- Possible antiplatelet activity — relevant if you take an anticoagulant or are approaching surgery.
- Measure, do not assume. Adding a supplement without measuring means you learn about an interaction from a symptom rather than from a number.
